Informed Consent, Client Responsibility, and Liability Release

Welcome to Reclaiming Hope Coach.

These services are designed to support nervous system regulation, capacity building, and gentle memory healing through a combination of coaching, somatic practices, and memory reconsolidation approaches.

This work is offered in both individual and group settings. While each experience is personal, the process often takes place within a shared learning environment where pacing, choice, and self-responsibility are central.

Outcomes vary based on many factors, including your level of participation, readiness, and willingness to engage with the practices offered. No specific results can be guaranteed.

Informed Consent

Purpose of Services

Services may include coaching, somatic practices, memory reprocessing approaches, hypnosis, guided imagery, progressive muscle relaxation, meditation, and grief-informed support. These approaches are intended to support personal growth, emotional regulation, and overall well-being.

These services complement but do not replace medical, psychological, or psychiatric treatment.

Sessions may be conducted via Zoom, in group settings, or through other virtual formats depending on the service provided.

Nature of Services and Expectations

Some services may involve hypnosis, memory-based techniques, somatic practices, and guided coaching processes.

Experiences vary from person to person. You may experience noticeable shifts, emotional responses, physical sensations, or minimal immediate change. At times, participation may bring awareness to uncomfortable emotions, memories, or sensations.

You acknowledge that outcomes are not guaranteed.

Coaching Philosophy

Coaching is intended to support growth, self-awareness, and forward movement. It focuses on current experiences, future direction, personal responsibility, and meaningful action.

This work is collaborative and depends on your honesty, participation, and willingness to engage.

Group Container, Participation, and Responsibility

All group sessions are held within a shared container designed to support safety, regulation, and respectful participation.

By participating, you agree to:

Participate within your own capacity

Monitor your internal state and take responsibility for your well-being

Pause, step back, or opt out of any exercise at any time

Respect the experiences, boundaries, and privacy of others

Contribute to a supportive and non-disruptive environment

Disruptive, harmful, or disrespectful behavior may result in removal from the session without refund.

You are responsible for seeking additional support, including medical, clinical, or crisis care, when needed.

Group Workshop Informed Consent

Group sessions may include shared space with other participants and, at times, optional opportunities to share personal experiences.

Participation is always voluntary and based on your personal readiness and choice. You are never required to share personal details.

While I maintain professional standards of confidentiality, I cannot guarantee that other participants will do the same. By participating, you agree to respect the privacy of others and not share information disclosed within the group.

Distinction Between Coaching and Therapy

Coaching is not therapy and is not a substitute for professional medical or mental health care.

Reclaiming Hope Coach does not provide diagnosis, treatment of mental health disorders, or crisis services.

Coaching services are intended for individuals who are able to participate with a level of self-awareness and personal responsibility. If additional clinical or therapeutic support is needed, referrals may be recommended.

Referral and Risk Awareness

If it becomes apparent that additional support is needed, you may be referred to appropriate licensed professionals.

Participation in these services may include risks such as emotional discomfort, increased awareness of distressing material, or temporary increases in stress.

Active participation and self-awareness are important to navigating these experiences.

Use of Hypnosis and Memory-Based Techniques

When hypnosis or memory-based techniques are used, you understand the scope and voluntarily agree to participate.

These approaches are not guaranteed to produce specific results and are not a substitute for medical or psychological care.

Certain medical or psychological conditions may not be appropriate for hypnosis or similar techniques. If you are unsure, you agree to consult with a qualified healthcare provider prior to participation.

Medical Responsibility

You agree not to change or discontinue any medical treatment, medication, or professional care without consulting your healthcare provider.

If you experience improvements or changes in your condition, you will seek medical guidance before making any adjustments.

Confidentiality and Communication

All information shared in individual sessions is held in confidence, except where disclosure is required by law, including situations involving risk of harm to yourself or others, abuse, or legal obligation.

Communication outside of sessions will be responded to as promptly as possible, but immediate availability is not guaranteed.

Confidentiality in Group Settings

Group sessions involve shared participation. While I uphold confidentiality, I cannot guarantee that all participants will do the same.

By participating, you agree to respect the privacy of others and not share any personal information disclosed within the group.

Emergency and Crisis Disclaimer

Reclaiming Hope Coach does not provide emergency or crisis services.

If you are experiencing a crisis, including thoughts of harming yourself or others, you agree to seek immediate support by calling 911, contacting a crisis hotline, or going to the nearest emergency room.

Emergency Contact Requirement

You agree to provide an emergency contact prior to participation. This is used only in situations where there is concern for your immediate safety.

Limitations of Online Services

Online coaching, messaging support, and group sessions are not a replacement for in-person therapy or crisis care.

These services may not be appropriate if you are in acute distress, unsafe environments, or experiencing severe mental health conditions requiring clinical intervention.

Client Responsibilities

You agree to:

Provide accurate and current information

Participate actively and communicate honestly

Take responsibility for your decisions and well-being

Engage with the process at your own pace

Respect all participants and the container

Arrive on time and prepared for sessions

A 10-minute grace period is allowed for late arrivals. After that, sessions may be considered missed.

Payment, Scheduling, and Attendance Policy

Payment

Payment is required in full prior to participation unless otherwise specified in a payment plan agreement.

All services, sessions, and programs are non-refundable except where explicitly stated.

Scheduling

All sessions must be scheduled through the provided booking system.

By reserving a session, you are committing to that time within a shared container.

Reminders

Reminder notifications may be sent prior to your scheduled session to support attendance. These may include email and/or SMS reminders.

Failure to receive or review reminders does not exempt you from attendance or cancellation policies.

Cancellations and Rescheduling

A minimum of 24 hours notice is required to cancel or reschedule a session.

Cancellations made with more than 24 hours notice may be rescheduled based on availability

Cancellations made within 24 hours of the session time are considered late cancellations and may not be rescheduled

Rescheduling is not guaranteed and depends on availability and program structure.

No-Show Policy

If you do not attend a scheduled session and have not provided at least 24 hours notice, the session is considered a no-show and will be forfeited.

Late Arrival Policy

Individual Sessions
A 10-minute grace period is provided. After 10 minutes, the session is considered missed and will not be rescheduled.

Group Sessions
Timing in group sessions is sensitive to the structure of the experience. Sessions may include guided regulation, somatic practices, or memory-based processes. Entering late during these phases may be disruptive to both you and the group.

Because of this:
• You are expected to arrive on time
• Late entry may be delayed or not permitted depending on where the group is in the process
• In some cases, you may be admitted during a natural pause or transition point
• Late entry is not guaranteed and will be determined based on the safety and flow of the session

Group Session Attendance

Group sessions begin and end on time.

By registering for a group session, you agree to:
• Arrive on time
• Remain for the full duration of the session
• Respect the flow and structure of the group container

Late entry or early departure may not be permitted once the session has begun, particularly during guided processes, and access will be determined based on the flow of the session.

Workshops and Programs

For workshops, series, or multi-session programs:
• Refunds are only available if cancellation occurs at least 5 business days prior to the start date, unless otherwise specified
• After this period, all payments are non-refundable
• Missed sessions are not refundable or transferable unless explicitly stated

In some cases, transfers to a future session may be offered at the sole discretion of Reclaiming Hope Coach and based on availability.

Unexpected Circumstances

If unexpected circumstances arise, you are encouraged to communicate as soon as possible.

While accommodations may be considered, they are not guaranteed and remain at the sole discretion of Reclaiming Hope Coach.

Recording and Intellectual Property

Recording of sessions, workshops, or materials is strictly prohibited without written consent.

All materials provided are for personal use only and may not be shared, distributed, or reproduced.

Unauthorized use may result in termination of services and legal action.

Termination of Services

Reclaiming Hope Coach reserves the right to terminate services if participation is not appropriate, safe, or aligned with these terms.

Liability Release

Assumption of Risk

By participating in coaching, hypnosis, memory reprocessing, somatic practices, and group or individual services, you acknowledge the inherent risks involved.

These may include emotional discomfort, activation, fatigue, or unexpected reactions.

Participation is voluntary and based on your personal readiness and choice.

Personal Responsibility

You acknowledge that you are responsible for your own well-being, choices, and actions during and after participation.

Release of Liability

You agree to release and hold harmless Reclaiming Hope Coach from any claims, damages, or liabilities arising from your participation.

Services are provided in good faith and are not a substitute for medical or mental health treatment.

Indemnification

You agree to indemnify and hold harmless Reclaiming Hope Coach from any claims resulting from your participation in services.

Electronic Signature Acknowledgment

By signing electronically, you acknowledge that:

Your signature is legally binding

Electronic communication satisfies written requirements

Electronic records are valid and enforceable

By clicking the "Agree & Sign" button below, you:

  1. Acknowledge and Consent to the terms and conditions outlined in this agreement, including all policies and provisions related to the services provided by Kathy-Reclaiming Hope.

  2. Understand that your electronic signature is the legal equivalent of your manual, handwritten signature on this document.

  3. Agree that after signing, Kathy from Reclaiming Hope will also sign the document to formalize the agreement between both parties.

  4. Will Receive a final copy of this agreement, featuring both your signature and Kathy's, for your records via email.
